The Quarterly Provider Capacity Survey closes tomorrow, Wednesday, September 28.

Completing this survey helps inform the Indiana Bureau of Rehabilitation Services on what supports your organization needs to ensure sustainable capacity and provide high-quality service to participants.

We ask that each employment service provider coordinate ONE submission per service location. Data you submit will be aggregated with other providers in your area and will not be linked to your organization.
