On behalf of the Indiana Bureau of Rehabilitation Services, we’re facilitating a Quarterly Employment Provider Capacity Survey.   We ask that each employment service provider coordinate ONE submission per service location.   

Why is this survey important?

Ultimately, this information will be used to make decisions on how to better support providers and how to optimize supports for program participants.

Data you submit will be aggregated with other providers in your area and will not be linked to your organization.  Combined information will be used to better understand the needs of providers across the state.
